Ammonia Poisoning
Ammonia poisoning is caused by an excessive amount of unionized ammonia (NH3) and is among the top killers of household fish. High levels of ammonia occur mostly in newer aquariums that are still establishing beneficial bacteria and a water cycle. It can also occur when a large number of fish are added to any already established aquarium at one time, filter malfunctions, and/or a severe change in water quality.

Actions
Ammonia is actually fairly easily treated, all you need to really do is insure you do a proper water change of about 25-50% of the water in the aquarium every month. You can also reduce feeding quantities and/or purchase a ammonia neutralizer.
